Patient Forms

For your convenience, we have made a number of forms available for patients and doctors to download. If you are a new patient to our practice, we encourage you to download and print out the New Patient Information Forms. Please fill these out in advance of your first visit and bring them with you.  This will minimize the time you spend in the waiting room when you get here. 

For referring doctors only: our Cataract Post-Op report form is now available online, along with the New Patient Referral form.  A supply of carbon-copy post op cataract forms are automatically sent to each doctor each time a patient is scheduled for surgery, but in the event you don't receive them then please download the form and make copies.  If you running low on referral forms for your office, please contact Terri Boxer for assistance or email her at terriboxer@bellsouth.net
